Referrals to the Graduate Programs Clinic
The Graduate Programs Clinic at UBC Dentistry accepts patient referrals for complex cases in orthodontics, pediatric dentistry, periodontics, endodontics and prosthodontics.
For referral to the Graduate Programs Clinic, print, fill out and fax back the general referral form: General referral form* (PDF)
If you have additional information in an endodontics referral, you may use this referral form: Endodontics referral form** (PDF)
* The general referral form is a print and fill form. Like a traditional paper form, this form is printed, filled out (by pen) and faxed to the Graduate Programs Clinic. Fax number is on the form.
** The endodontics referral form is fill and submit form via e-mail. The form is filled out on screen and automatically e-mailed as an attachment to the clinic once the submit button on the form is pressed. You may attach radiographic images to the e-mail message and send them with the form. You will need a desktop e-mail program, such as Outlook, Outlook Express, Apple Mail, etc. Web-based e-mail (Hotmail, Gmail, Yahoo! Mail, etc.) will not work.
